The most common reason is a filter, whether a person is using Boxbe or another service that filters out potential spam or the email provider themselves. 

Historically, the email providers that have been the strictest are AOL, Earthlink, Comcast, SBCglobal and some others.

Hotmail/Live has become more difficult in the last six months.

We do have a high deliverability rate and the way that we ensure this is a monitoring of blacklists, spam prevention and implementation of methods to increase our reputation with email providers.
